Informed Consent and Hold Harmless Agreement: Creatures of Consciousness | Madeline Sol ThetaHealing & Consciousness Sessions and Classes 

Please read these terms carefully before using our service.

  1. I have received information that the ThetaHealing Technique is a method for assisting the body in its natural healing process. It is a complementary therapy not intended to replace any currently prescribed medical treatments as ordered by my physicians, nor any other medical care I have been advised to seek by them.  

  2. I have been informed that Madeline Sol is a certified ThetaHealing Technique practitioner and will neither diagnose, nor prescribe for any condition that I may have, nor does she make specific claims regarding results from ThetaHealing sessions that I receive. I have also been informed that Madeline Sol is not licensed to practice medicine in the state of California or any other state or country.  

  3. I have been encouraged to consult a licensed medical practitioner for any physical or mental complaints that I may have.  Treatment/s will not replace conventional medical diagnosis or treatment.  I will continue taking medication prescribed by a licensed medical physician and will continue to follow his/her instructions.

  4. I understand that I may stop treatment at any time, either during or outside of each session and that I am responsible for voicing any concerns regarding treatment as they present themselves, so that the practitioner, Madeline Sol, may address them in a timely and conscientious manner.  

  5. Some of the indications for a ThetaHealing session include, but are not limited to:

    • Reduction in pain, stress and anxiety

    • Ability to make clearer decisions with ease

    • Getting in touch with your natural psychic abilities and learning to trust your intuition

    • Becoming more aware of your role in creating your life

    • Support for the body’s natural healing process and a sense of well-being

    • Facilitation of wound healing

  6. I have been informed that all client information and records are treated in a confidential manner.  My experiences during these sessions are confidential and subject to the usual exceptions governed by state and federal laws and regulations.

  7. By electronically marking the checkbox to indicate consent, I hereby waive and release Madeline Sol from any and all liability past, present, and future relating to her services, whether they be onsite at the home of the client signing this consent or at another location.  

  8. Except in the case of gross negligence or malpractice, I or my representative(s) agree to fully release and hold harmless Madeline Sol from and against any and all claims or liability of whatsoever kind or nature arising out of or in connection with my session(s).

  9. My questions have been answered to my satisfaction regarding Madeline Sol’s training and experience with ThetaHealing and what I might expect from my session/s.

  10. I have read and agree to the terms above.

Consciousness Healing and Coaching Sessions and Classes:

Waiver, Release, and Assumption of Risk:

This form is an important legal document. It explains the assumption of any and all risks in deciding to follow the advice or insight, or use of products, from intuitive consultant, facilitator and energy healer Madeline Sol, who operates as a spiritual advisor, mentor, and facilitator of consciousness through Creatures of Consciousness.

It is critical that you read and understand it completely.

Confidentiality:

The identity of the client and our ongoing work together will be kept strictly confidential. Information about our work will only be released with the client’s written permission, or if required by court order. The following exceptions will apply:

1. There are a broad range of events that are reportable under child protection statutes. Physical or sexual abuse of a child will be reported to Child Protective Services. When the victim of child abuse is over age 18, there is no legal mandate to report it unless there is reason to believe that there are minors still living with the abuser who may be in danger of being abused. Elder abuse is also required to be reported to the appropriate authorities.

2. If the client is at imminent risk to theirself or someone else or make threats of imminent violence against another person, appropriate action will be taken.

‍Waiver, Informed Consent, and Covenant Not to Sue:
‍I hereby acknowledge I have volunteered to participate in a paid or unpaid session, class, workshop, service or program with Madeline Sol and/or an associate available through Creatures of Consciousness to include, but also may not be limited to, any and all services provided, such as energy healing, intuitive coaching, consciousness facilitation, guidance, classes, product offerings and workshop attendance. In consideration of the Creatures of Consciousness agreement to instruct, assist, advise, or train me, I do here and forever release and discharge and hereby hold harmless the Company and its respective agents, heirs, assigns, contractors, and employees from any and all claims, demands, damages, rights of action or causes of action, present or future, arising out of or connected with my participation in a session or purchase or any program WITHOUT LIMITATION, WHICH MAY OCCUR AS A RESULT OF following advice tendered and released or training rendered or use of facilities during a session or event.

I recognize that in no way does any member or representative of Creatures of Consciousness provide legal, medical, or therapeutic advice and it is my responsibility to secure such advisement. Your sessions at Creatures of Consciousness are not a substitute for counseling, psychotherapy, mental health care or substance abuse treatment. I acknowledge and agree that I assume the risks associated with any and all activities, offerings, products, services, classes and/or programs in which I participate or purchase.

As the client, I am aware that if I display signs of psychosis, substance use disorder or symptoms indicative of a mental health disorder, I will be referred to a psychotherapist for counseling or therapy, or to a crisis response center for an emergency psychiatric assessment.

I acknowledge and agree that no warranties or representations have been made to me regarding the results I will achieve from any insight, guidance, product or program. I understand that results are individual and may vary.

I further acknowledge that if granted permission to audio record my sessions with Madeline Sol, they are for personal use only. I understand that I MAY NOT share my recordings privately or publicly in any form, as to do so would violate the terms of this agreement, breach trust, and break a code of privacy. Any unauthorized use, sharing, broadcast or release, personally or publicly, in any form, of a recorded session, is strictly prohibited and would be considered an illegal act and punishable by law.

I ACKNOWLEDGE THAT I HAVE THOROUGHLY READ THIS WAIVER AND RELEASE AND FULLY UNDERSTAND THAT IT IS A RELEASE OF LIABILITY. BY ELECTRONICALLY MARKING THE CHECKBOX TO INDICATE CONSENT, I AM WAIVING ANY RIGHT I OR MY SUCCESSORS MIGHT HAVE TO BRING A LEGAL ACTION OR ASSERT A CLAIM AGAINST MADELINE SOL OR CREATURES OF CONSCIOUSNESS FOR NEGLIGENCE OR THAT OF ITS EMPLOYEES, AGENTS, OR CONTRACTORS.
